Supporting Documents
Provided below is a list of Supporting Documents, in the order the student will need to upload them while completing the application form. Scan, and save the following documents on your computer, which support the information you provide in the application form, prior to beginning to fill out the application form.
The file size of each Supporting Document scanned, must not exceed 1 MB (1024 KB) and it must be saved in .jpg format.

| 3 | Fees Paid Receipt | The course name, the semester or year for which the fees are paid for, and the fee amount |
| 4 | Employer's Letter, I. Tax Return or Proof of Income of the Earning Members of the Family | Income to determine the financial affordability to pay the education fees |
| 5 | Student's Bank Account Book Page | The bank details |
| 6 | Student’s scanned signature Must be within a block size: 1.5" (width) x 1" (height) |
Indicates that the Student accepts the responsibility for the commitment made. |
| 7 | Parent’s scanned signature Must be within a block size: 1.5" (width) x 1" (height) |
Assures that the Parents are also responsible for the commitment the student makes. |
NOTE:
If any of the above required documents is not submitted or the information provided in the application does not agree with the Supporting Documents, the student shall be considered ineligible to receive the scholarship and his/her application shall be disapproved.
